Admission procedures
Your admission will be determined and scheduled by a doctor based on your medical condition assessed through consultation and bed availability. You will be informed of the rough date of your admission when you book for admission.
The hospital calls patients booked for admission to notify them of their admission date/time. In principle, the call will be made between 13:00 and 15:00 on the day before their admission (if admission day falls on a Monday, the call will be made on the preceding Friday). Please be prepared so that you can get admitted on the designated date.
Please undergo admission procedures at the Admission Desk on the first floor by the designated time. When procedures are completed, a nurse from the ward you will be staying at will guide you to your room.
You will need the following items for your admission procedures.
- Admission Application Form (The form is inserted between pages of the Guide to Your Hospital Stay given to you upon booking your admission.)
- Health insurance card, medical certificate(s) (if you have any)
- Patient ID card (plastic card issued by this hospital)
- Seal
- Patient Information Form (The form is inserted between pages of the Guide to Your Hospital Stay given to you upon booking admission.)
- Proof of Discharge (Please bring Proof of Discharge if you have received it from this or another medical institution.)
- Donor card, if you have one. Please present it to your attending doctor or nurse upon admission.
- Medicine pocketbook (okusuri techo) or a description of medication
How to fill in Admission Application Form
Please fill in the Admission Application Form by following the instructions below.
Applicant:
In principle, write the name of the patient who is to be admitted. If the patient is a child, an elderly individual, or an emergency patient accompanied by a parent, guardian or family member, write the name of the accompanying person.
Guarantor:
Write the name of a person who can be contacted, and preferably, does not live at the same address as the patient.
Name of Patient:
Write the name of the patient who will be staying at this hospital.
Life as inpatient
Items needed for your hospital stay
Please bring the following items on the day of your admission.
- Sleepwear: 2–3 sets
- Underwear: 2–3 sets
- Bath towel, face towel: 2–3
- Indoor shoes that do not slip: 1 pair
- Toiletries (toothbrush, toothpaste, cup, soap, shampoo, conditioner, shaving set, etc.)
Wet tissues (if needed): 1 set -
Tissues: 1 box
The above daily necessities can be purchased at the store in the hospital.
Sleepwear
Patients undergoing surgery or bed-ridden patients are required to wear kimono-style sleepwear or front-open-type pajamas. Other patients are free to wear any type of sleepwear. - For patients undergoing surgery, a nurse will provide information on what they additionally need after admission. (E.g., Kimono-style sleepwear, abdominal/chest/T-shaped bandage, paper diapers, etc.)
- Patients who use paper diapers in their daily life should bring them in. We may also ask paper diapers and wet tissues to be brought in if patients are bedridden or need support to use the restroom.
- Chopsticks and tea cups are supplied by the hospital, but you are free to bring your own.
- Electric razors can be brought in without permission. As for other electronic appliances such as mobile phones and computers, doctor’s permission is required. Permission will be given if your doctor deems them to have no negative impact on your treatment. All your electronic appliances must be charged with batteries or charged at home.
- You are not allowed to bring in scissors, knives or other sharp objects. If you need to use such objects, please contact a nurse for approval.
Patient rooms
The standard room at this hospital is a four-patient room, which is equipped with beds, bedside cabinets with a TV, a toilet, a wash basin and personal lockers. To watch TV, a prepaid TV card needs to be purchased. Please bring in earphones, which can also be purchased at the store in the hospital.
Special rooms
Patients can choose a fee-based special room if there is vacancy. If you wish to use a special room, please let us know when undergoing admission procedures.
Patients may be requested to transfer rooms during their hospital stay when medical conditions of other patients change or for other reasons. We appreciate your cooperation.

Other points to note
Mobile phones should only be used within designated areas.
Electronic devices such as computers and audio players are not allowed to be brought into patient rooms.
Earphones should be used when watching TV in patient rooms. (TVs can be watched with prepaid TV cards in patient rooms.)
To protect your privacy, we do not take outside calls on your behalf. However, we will inform you of any calls made to you.
Each ward is equipped with a coin laundry.
Inpatient hospital fees
At this hospital, all room fees are covered by health insurance except for special room fees.
Inpatient hospital fees must be paid on the 10th of each month (fees for the preceding month) and/or when patients are discharged.
For an estimate of your inpatient hospital fee, please ask the Medical Affairs Section through a nurse.
If you have any problems or concerns, please consult the Patient Support Center.
The hospital does not take guarantee deposit upon admission.
An ATM is located at the store in the hospital.

We accept UC, VISA, MasterCard, JCB, and American Express (including co-branded cards)
24 hours a day (including at the emergency reception on holidays and during nighttime).
(2) Debit cards
Cash cards issued by banks and Japan Post Bank can be used as debit cards up to the balance of your account.

Saturday/Sunday/holidays 9:00‒19:00
(3) E-money
We accept QUICpay, iD, and prepaid transportation cards (e.g., Suica, PASMO) 24 hours a day including at the emergency reception on holidays and during nighttime.
3 Accepted payment arrangements
Payments can be made in a single installment, multiple installments, or revolving payment. Payment with debit cards and prepaid transportation cards must be made in a single installment.
4 Applicable services
The above applies to all payments for inpatient and outpatient care services provided from April 2006.
Visiting patients
Visiting hours
General wards / SCU (Stroke Care Unit) 7:00 to 21:00
ICU (Intensive Care Unit) 10:00 to 21:00
When visiting a patient
- When visiting between 8:00 and 18:00, fill in the Visitor Form at the Inpatient Visitor Reception of the General Reception on the first floor. Wear the visitor’s badge given to you.
- When visiting in hours other than the above, visit the security guard at the emergency entrance on the first floor.
- Call on the clerk or nurse at the ward before visiting a patient.
- The ward dining room can be used to meet patients, except during meal times.
Rules for visitors
- To prevent mutual infection between patients and visitors, do not bring children under 12.
- Do not engage in acts that disturb other patients.
Post-discharge medical care
As an acute care hospital, we collaborate with local medical institutions to widely accept patients who require acute care. When a patient is no longer in an acute phase and is maintaining a stabilized condition, we hand him/her over to the medical institution that initially referred him/her to this hospital. Patients who are in a stabilized condition but still require ongoing hospitalization will be transferred to our partner hospital to receive ongoing medical care.
Our doctor or other staff will provide you with full explanation on your medical care after discharge. Please do not hesitate to discuss your concerns with a doctor if you have any.
Inpatients who were not referred to this hospital by another medical institution, such as those admitted due to an emergency, will be referred to our partner hospital after discussing their choice.
